Prime Longevity — Website Privacy Policy

Last updated: October 7, 2026

1. Introduction and Scope

Prime Longevity LLC ("Prime Longevity," "we," "us," or "our") respects your privacy. This Privacy Policy explains how we collect, use, share, and protect personal information when you visit getprimelongevity.com and its related pages, forms, and online features (the "Site"), or when you communicate with us by email, phone, text message, or social media before becoming a patient.

Your health information is handled separately. Once you become a patient, the medical and health information we create or receive in connection with your care is "protected health information" under the Health Insurance Portability and Accountability Act (HIPAA) and Florida law. It is governed by our Notice of Privacy Practices, which is provided at intake and available on request. If this Privacy Policy and the Notice of Privacy Practices conflict as to protected health information, the Notice of Privacy Practices controls.

By using the Site, you agree to this Privacy Policy and to our [Terms of Use]. If you do not agree, please do not use the Site.

Definitions. In this Policy, "personal information" means information that identifies, or could reasonably be linked to, you or your device. "Usage data" means information collected automatically when you use the Site. "Service providers" are companies that process information on our behalf under contract.

2. Information We Collect

Information you give us. When you submit an inquiry, apply for membership, sign up for our newsletter, contact us, or book a consultation, we may collect:

Contact details

Examples: Name, email address, phone number, country, mailing address

Areas of interest
Examples: Services you select (for example, diagnostics, cellular regeneration, peptide therapy, IV therapies, hormone optimization, sexual wellness, regenerative aesthetics, hair restoration, membership, telehealth)

Messages
Examples: Anything you write in free-text fields, emails, texts, or direct messages

Membership applications
Examples: Information you provide when applying for a membership tier

Communication preferences
Examples: Your email and text-message opt-ins and opt-outs

Your selection of a service of interest may reveal information about your health. We treat it confidentially, use it only to respond to your inquiry, and do not use it for third-party advertising. Please do not send detailed medical history, test results, or images through website forms, email, or social media. We will collect clinical information through secure intake channels.

Information collected automatically. When you visit the Site, we and our service providers may automatically collect usage data, such as your IP address, browser type and version, device type and identifiers, operating system, pages viewed, time and date of visits, time spent on pages, referring website, and general location inferred from your IP address. See Section 4.

Information from third parties. We may receive information about you from:

• Our website platform, scheduling, and communication providers;

• Cherry or other patient-financing partners, such as whether you applied or were approved (we do not receive your full credit report);

• social media platforms when you interact with our pages or send us messages;

• people who refer you to us; and

• other healthcare providers or laboratories, with your authorization, once you become a patient (governed by our Notice of Privacy Practices).

3. How We Use and Share Information

How we use it. We use personal information to:

• respond to inquiries and schedule consultations;

• review membership applications and manage memberships;

• send appointment confirmations, reminders, and service messages;

• send newsletters, offers, and event invitations if you opt in;

• operate, secure, maintain, and improve the Site;

• understand how visitors use the Site, in aggregated or de-identified form;

• detect and prevent fraud, abuse, and security incidents; and

• comply with legal obligations, enforce our Terms of Use, and protect our rights and the safety of patients, staff, and others.

We do not sell your personal information, and we do not share it with third parties for their own marketing.

How we share it. We share personal information only as follows:

• Service providers. Companies that host the Site, provide email, text messaging, scheduling, forms, payment, analytics, and IT services, under contracts that limit their use of the information to serving us. Where a provider handles protected health information, we require a HIPAA business associate agreement.

• Financing partners. If you choose to apply for financing, information needed for your application is shared with that lender (currently Cherry). The lender's own privacy policy governs its use of your information.

• Affiliated clinicians. Licensed providers who work with Prime Longevity, to respond to your clinical inquiry.

• Business transfers. In connection with a merger, acquisition, financing, reorganization, or sale of all or part of our business, subject to this Policy and applicable law.

• Legal and safety. When we believe in good faith that disclosure is required by law, subpoena, or court order, or is necessary to protect the rights, property, or safety of Prime Longevity, our patients, or others.

• With your consent. For any other purpose disclosed to you when we ask for your consent.

We may share aggregated or de-identified information that cannot reasonably identify you for any lawful purpose.

4. Cookies, Analytics, and Tracking Technologies

The Site, which is hosted on the Wix platform, uses cookies, pixels, local storage, and similar technologies. Cookies are small files placed on your device that let a website recognize your browser.

Strictly necessary

Duration: Session or persistent

Preferences
Purpose: Remember your choices, such as cookie consent and language
Duration: Persistent

Analytics
Purpose: Understand how visitors use the Site so we can improve it, using aggregated statistics
Duration: Persistent

Marketing (only with your consent, where required)
Purpose: Measure the effectiveness of our advertising
Duration: Persistent

Health-related pages. We configure our tracking technologies so that they do not send information about the services you view or select, or the content of your inquiries, to advertising platforms.

Your choices. You can manage cookies through our cookie banner (where available) and your browser settings, which let you block or delete cookies. Blocking strictly necessary cookies may cause parts of the Site to stop working. Some browsers send a "Do Not Track" signal; because there is no common standard for these signals, the Site does not currently respond to them. We honor Global Privacy Control signals where required by law.

Third-party features. Links to Google Maps, Instagram, and LinkedIn take you to services operated by those companies, which may collect information under their own privacy policies.

5.Email and Text Message Communications

Email. If you subscribe to our newsletter, we will send updates, offers, and news about longevity and our services. Every marketing email has an unsubscribe link. Even after you unsubscribe, we may still send messages about your appointments, inquiries, or membership.

Text messages. If you check the text-message consent box or otherwise opt in, you agree to receive recurring automated marketing and appointment-related texts from Prime Longevity at the number you provide. Consent is not a condition of purchase. Message frequency varies, and message and data rates may apply. Reply STOP to opt out at any time or HELP for help.

We keep a record of your consent, including the date, time, and wording you agreed to. Mobile numbers and text-messaging consent are never shared with or sold to third parties or affiliates for their marketing or promotional purposes. We share them only with the service providers that deliver our messages.

6.Security, Retention, and Your Rights

Security. We use reasonable administrative, technical, and physical safeguards to protect personal information, including encryption in transit and access limited to personnel who need it. No website, transmission, or storage system is completely secure, so we cannot guarantee absolute security. If a breach affects your personal information, we will notify you as required by Florida law (Section 501.171, Florida Statutes) and, for protected health information, by HIPAA.

Retention. We keep personal information only as long as needed for the purposes in this Policy, including to respond to you, maintain our business records, resolve disputes, and enforce our agreements, or longer when the law requires it. Inquiry and marketing records are reviewed periodically and deleted or de-identified when no longer needed. Medical records are retained as Florida law requires and as described in our Notice of Privacy Practices.

Where data is stored. Prime Longevity is located in Florida, and our service providers store information in the United States. If you access the Site from outside the U.S., your information will be transferred to and processed in the U.S., where privacy laws may differ from those in your country.

Your choices and rights. Depending on where you live, you may have the right to:

• request access to, or a copy of, the personal information we hold about you;

• ask us to correct inaccurate information;

• ask us to delete your information, subject to legal and record-keeping exceptions; and

• opt out of marketing emails and texts at any time.

To make a request, contact us using the details in Section 7. We will verify your identity before acting on a request and respond within the time required by applicable law. We will not discriminate against you for exercising your rights. Rights over protected health information, including access to and amendment of your medical records, are described in our Notice of Privacy Practices.

Children. The Site is intended for adults 18 and older. We do not knowingly collect personal information from children under 18 through the Site. If you believe a child has given us personal information, contact us and we will delete it. Care for minors, where offered, requires a parent or legal guardian and is handled through our clinical intake process.

Third-party websites. The Site links to websites and services we do not control, including social media, mapping, and financing platforms. We are not responsible for their privacy practices. Please review their privacy policies before providing them with information.

7. Changes and Contact

Changes to this Policy. We may update this Privacy Policy from time to time. We will post the updated version on this page with a new "Last updated" date. If we make material changes, we will also notify you by email or by a prominent notice on the Site before the changes take effect. Your continued use of the Site after the effective date means you accept the updated Policy.
